To establish a diagnosis with one of Harley ENT Practice’s consultants please write down, as accurately as possible, your medical problem so that we can refer to the symptoms during your consultation. Describe the character, location, intensity, duration and time of the beginning of your symptoms. You should also provide a detailed list of any medications you are or have been taking and inform us of any medical allergies.
